Off-Road Equipment Replacement Application Form
The online form below can be completed as an alternative to completing the printable Off-Road Equipment Replacement Application Form (we're testing this out - thanks for your patience). Before completing this online form, please read the Instructions and Program Criteria (Pages 1 and 2) contained in the printable application.
Please email the following items to jmandly@bcaqmd.org to support your application:
- Proof of equipment ownership (Bill of sale, tax records, equipment insurance records, etc.) showing ownership for at least four (4) years.
- Equipment usage records covering past 24 months (hour meter readings, employee logs, fuel logs, or detailed breakdown of hours of use, etc).
- Itemized quote for new equipment with warranty information.
- Executive order (emissions) or engine family number for new engine. Dealer can assist.
- Proof of General Liability and Workers Compensation (if applicable) insurance.
- DOORS Fleet Compliance Certificate (if subject to In-Use Off-Road Regulation – equipment used 100% for Ag is exempt).
By e-signing below and submitting this application, you are certifying that you understand and acknowledge grant requirements, you certify that the applicant is currently in compliance with all federal, State, and local air quality rules and regulations at the time of submittal, and you certify under penalty of perjury that the information in the application and attachments is accurate and true.
